Roadside Assistance is commonly offered as an optional paid service by your rental company. Details on various programs are listed below.

Keep in mind that you can either sign up for roadside protection at the beginning of your rental (when you do your contract) or you may automatcially "opt-in" to the purchase of this service if you use it during your rental -- for example: calling the provided number to request help for lost keys, towing, fuel delivery, etc.
It's always best to carefully read your rental contract and discuss this type of program with your rental agent before leaving the rental counter. Find out what is included and what is not covered (ie ask about flat tires, lost keys, etc)
Note: Car Rental roadside protection programs are void and will not apply if, at the time of an incident, the vehicle is being used in such a way that is contrary to and violates the terms and conditions on the rental agreement. If this happens, help is still provided, but extra charges will be applied.
Alamo & National
This service is only offered in the 50 Contiguous United States and in Canada.
The average cost for Alamo's car rental roadside protection is $4.99 per day in the US and $6.99 per day in Canada.
For rentals picked up in California, Kansas, Nevada and New York, Roadside Plus does not include replacement of lost keys or remote entry devices.
I could find no cost information for National. Renters should check at the counter.

EnterpriseAccording to Enterprise.com (opens in a new window), Roadside Protection can be purchased at participating locations for help with things such as lost keys, lockouts and fuel outages.
HertzAccording to Hertz.com (opens in a new window) the number for Roadside Assistance will be provided on your contract and in your vehicle.
I could find no information on Hertz.com about there being a charge for this service (and it was not given as a booking option in a rate quote), but I did find a statement while researching that in the case of lost keys, Hertz may charge you for the cost of delivering the keys or towing the car to the nearest Hertz location.
